Extruder motor doesn't turn

I am using a Mega/Ramps 1.4 board
New build
I have gotten all the other steppers working and tuned but the Extruder has me stumped
When I position it manually from the menu I get no movement. 
 
The motor works great on other drivers.
The driver works great on X/Y/Z

I wondering if there is an interlock or lockout I am overlooking.

Any thoughts on what I am doing wrong?

    Send M302 S1 to allow cold extrusion. Otherwise extruder will only turn when hot. Also make sure dry run mode is not enabled because your thermistors are still missing or something like that. This could also enable dry run mode.
