Check in host/server the output power. I guess you have limited max. PWM to a value < 255 and this prevents the power needed to heat it further.

There are 2 max pwm values. If you really see 100% in host temperature then your heater is too weak. Fan cools more then you can heat. Normally this is not the case for full metal hotends that need extra cooling and it is the pwm. Also check eeprom values.
